Insurance Information

In general, we accept all major dental insurance plans, and are providers for many PPO's.  We are not providers for any HMO plans.  However, we can still see you and provide services if requested by your primary dentist.  We also accept many medical insurances, as many of our services are may be covered under your plan.  

Medical and dental plans in the market place are too numerous and varied to allow us to know details of all of them. Your particular plan may or may not cover the full costs of your surgical treatments. As well, there may be certain procedures that are not covered through your dental plan. These factors are beyond our control.

Upon completion of your examination and consultation, you will be provided with an estimate of your treatment costs. We will also estimate what we believe your plan may cover towards the total costs based on the information that has been provided to us. By request, a predetermination of benefits can be completed by our office prior to treatment.

Insurance plans typically will not pay for 100% of our fees.  Most plans have copays and cover only a percentage of the cost for the service rendered. You are expected to pay for any portion beyond what your plan covers on the day of surgery.

Important: If we do not receive payment from your insurance company within 45 days after surgery, you will be billed directly for the full fee. Please note predetermination is not a guarantee of payment - due to plan maximums or coverage cancellation.

Patient Responsibility

We are not responsible for yearly plan maximums being reached by the patient/insured or coverage cancellation prior to the date of service. It is the patient's responsibility to check with their dental plan regarding these matters prior to the day of surgery.
